These worksheets are updated frequently during the first half of December, as districts/counties report. Please be sure you are working on the latest version by downloading a new copy prior to entering data for reporting.

Submit Mill Levy Certification Data to CDE via State Equal no later than December 15, 2025
Submit Mill Levy Certification Form and all Certifications of Assessed Valuation no later than December 15, 2025 (worksheet submittal is optional)
DOLA Mill Levy Public Information Form
Districts are required to submit the DOLA Mill Levy Public Information Form to their counties, using whatever process each county's assessor requires. The counties then relay that information to DOLA.
Although CDE does not administer or receive this form, we have assembled some resources you may find helpful for completing it:
