2024-25 Health Data Report
Open April 1, 2025 - May 23, 2025

 

The CDE School Health Data Report is a voluntary survey for school nurses. 

Please submit ONE report per district. 

• Connect with your school nurse lead to identify who will be reporting the data. 
• If you are a charter school under the authority of a school district, please check with the district school nurse lead to see how your data will be reported to ensure data is not double-reported. 
• Charter schools under the authority of Charter School Institute will report their individual school's data. 
• Private schools will also report their data individually by school.
