File a Special Education State Complaint
The CDE developed a Model State Complaint Form to help ensure that state complaints include all required information. The Model State Complaint Form provides instructions and examples of how to describe allegations (problems or concerns) and the supporting facts for each allegation. Use of the Model State Complaint Form is optional and is not required to file a state complaint.

State Complaint Procedures
The CDE adopted written procedures, consistent with 34 C.F.R. § 300.151, for resolving state complaints filed by individuals or organizations who believe a public agency is not following special education law.
