Publication Transmittal Form for Print Documents This form should be used when sending documents to the State Publications Library. The form can be filled in on-line and then printed for submission with the documents being sent. At this time the form cannot be submitted electronically.

Notify the State Publications Library when digital publications have been released by sending the URL or PDF of the new document to spl@cde.state.co.us.
